Tananarive-East Coast (TCE) connects Tananarive and Toamasina via Moramanga (372 km). The Sanjavavy-Lohariandava railway bridge, destroyed by Cyclone Enawo in 2017, was rebuilt the same year. Rural communities are only served by the TCE railway.

The Moramanga-Lac Alaotra (MLA) line between Moramanga and Ambatondrazaka is 142 km long; Tananarive-Antsirabe (TA – 159 km).

Fianarantsoa-Côte Est (FCE) between Fianarantsoa and Manakara (163 km) is a legendary railway line in the truest sense of the word. Its construction lasted from 1926 to 1936, with the aim of opening up the east of the country in order to promote the export of Malagasy agricultural products to Europe. The FCE has 18 stations, 48 tunnels (the longest of which measures 1,072 m), 67 bridges, Four large viaducts and... an airport! Indeed, the line passes through the runway of Manakara Airport. The route along the east coast from Manakara to Fianarantsoa has a vertical drop of 1,200 m with a gradient of up to 3,661 ft3 in (3,661 m), one of the steepest in the world.

The first three are operated by Madarail (Vecturis group), which has undertaken a major upgrade program with the aim of restoring passenger and freight traffic across the entire network.

Since the Antsirabe-Fianarantsoa section was never completed, the Fianarantsoa-Manakara line is isolated from the rest of the network.

Passenger trains currently run between Moramanga and Toamasina. This section of the line was opened as a priority because it serves locations that are difficult to access by road.

The Fianarantsoa-Manakara line is also open to passengers.
